LOGIN INSTRUCTIONS:
New to the MD Anderson Online Credentialing Application Portal?
1. Enter your Email Address, this is the same email used to receive credentialing communications from Medical Staff and Credentialing Services.
2. Click on the First Time Login to receive an email with instructions to reset the account password. Please be advised that the email sent to reset passwords will expire in 72 hours.
PASSWORD SECURITY REQUIREMENT
- Need to be at least 12 characters
- Must contain Alpha and numeric characters with mix-cased Letters (at least one uppercase letter)
- At least one special character such as !@#$%*
- After 5 unsuccessful login attempts, your account will be locked. Please call Medical Staff and Credentialing Services at 713-794-1011 or email MedicalStaffOffice@mdanderson.org for assistance to reset your password.
After password is reset, please click Go to Login Page to return to the main page.
On main page, please enter your email address again and the new password you set up to launch the online credentialing application.
FORGOT PASSWORD OR NEED TO RESET EXPIRED PASSWORD PLEASE FOLLOW INSTRUCTIONS BELOW
1. If you need to reset your password, please enter email used to start online application in the Email Address field then click on Forgot Password.
2. An email with instruction to reset password will be sent to your email. Click on the link to find instruction to set New Password. Please be advise that the email sent to reset passwords will expire in 72 hours.
3. Your new password must follow the password security requirement listed above.Please do not use your old password (less than 2 years ago) when resetting password.
4. After resetting password, please go to main page to log back into your online application.

- Desktop or laptop computer (cell phone, Mac Computers, and other Apple products, such as an I-Pad, are not compatible with the application)
- Need to view a PDF file with Adobe Reader 9 or greater
- Google Chrome browser
